A user of Homeopathy who is taking help from someone (who can use computer / smart phone) to share my experience with Homeopathy.

I will share my / my child’s experience truthfully. I authorize the representative of the site to contact me to clarify my / my child’s input if necessary. If the representative of the site finds my / my child’s input inappropriate, irrelevant, questionable or offensive to any individual or the group then I understand that my / my child’s input may be rejected.

My (first name, last name, contact tel. # and contact email) will be used ONLY to contact me by the representative of this site AND it will not be open to the public. The connection of my name with my Homeopath will not be open to the public. All other information will be open to the public
